    This plan is based on a whole-system approach that addresses shared risk factors and the determinants of health across chronic disease rather than addressing specific diseases on their own. A simple one percent drop in preventable health conditions results in saving millions of dollars in health care costs. There are multiple factors that impact chronic disease prevention and control including socio-demographics, lifestyle, the built environment, employment, and community design, many of which point to the need to address larger systemic inequities in access, representation, power, and income that influence the social determinants of health. John Linney developed and authored this plan after facilitating a year-long statewide process.
